Ozark Oral & Maxillofacial Surgery
Oral and maxillofacial surgery requires up to 6 additional years of hospital based surgical and anesthesia training. As oral and maxillofacial surgeons, Ozark OMS manages a wide variety of problems relating to the mouth, teeth and facial regions in Rogers, AR. Ozark OMS practices a full scope of oral and maxillofacial surgery with expertise ranging from cosmetic facial surgery to corrective jaw surgery and wisdom tooth removal. We can also diagnose and treat facial pain, facial injuries and TMJ disorders, and performs a full range of dental implant procedures.
The staff is trained in assisting with I.V. sedation within our certified state of the art office setting. Patients are continuously monitored during and after surgery. In addition, general anesthesia provided by a certified anesthetist is available within our facility.
The surgical staff at Ozark OMS are experienced, certified oral and maxillofacial surgical assistants, who assist in administration of I.V. sedation and surgery. We also have a trained nurse to assist in surgery. Our business staff are informed administrative personnel, well-versed in health and insurance policies and our surgical staff are CPR certified.
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any X-rays if applicable
- A list of medications you are presently taking
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but that must be implemented prior to the actual procedure.

Ozark OMS is open Monday - Friday from 8 am until 5 pm. We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.
We try our best to stay on schedule to minimize your waiting. Due to the fact that Ozark OMS provides surgical services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

Meet Dr. Lewis
Dr. Robert Lewis founded Ozark Oral and Maxillofacial Surgery in 1980. He graduated dental school at the University of Tennessee in 1976. He completed his residency in Oral and Maxillofacial surgery at the University of Tennessee Health Science Center.
Meet Dr. Rockacy
Dr. Jeffrey Rockacy attended dental school at the University of Pittsburgh where he graduated in 1999. He then obtained his medical degree from the Mayo Clinic in Rochester, MN. He then completed his specialized training in Oral and Maxillofacial Surgery also at the Mayo Clinic.
Meet Dr. Baker
Dr. David Baker attended dental school at the University of Tennessee where he graduated in 2007. He then attended the University of Texas Medical Branch in Galveston, Texas where he completed his Oral & Maxillofacial Surgery Residency in 2011. He has been with Ozark Oral and Maxillofacial Surgery since the summer of 2011.
Meet Dr. Baber
Dr. M. Andrew Baber, a native Arkansan, has been practicing oral surgery in Arkansas since 2007. He received his undergraduate degree at Ouachita Baptist University; and earned his doctoral degree in dentistry at the University of Tennessee Health Science Center, College of Dentistry in Memphis. Following dental school, Dr. Baber spent four years completing his oral and maxillofacial surgery training at Virginia Commonwealth University Medical Center in Richmond.
Besides providing various oral and maxillofacial surgical procedures, Dr. Baber is also devoted to caring and compassionate community service, shown by his involvement with local free clinics, the Arkansas Mission of Mercy, donated dental services and the Friends of Barnabas Foundation Cleft Lip and Palate Mission Team in Honduras. Dr. Baber resides in Rogers, Arkansas with his wife, Susanne and two children. He enjoys spending his leisure time biking, hiking and playing guitar.
